Floral Park Little League Registration

Late Registration has been extended until March 1, 2010

PLEASE NOTE:

· Parents or Legal Guardians need to fill out legibly an application for each child registrant, a medical release form for each child (which will be regarded as confidential information, and one Parent Code of Conduct for the family. If you plan to volunteer or help coaches, you will also need to fill out a Volunteer Application and either copy or show a valid Driver's License .

· Late Registrations fee $25.

· Children registering for the first time will need to bring proof of residency (e.g. a recent utility bill) and a birth certificate.

· Please send all material, with copies of appropriate documentation to:

Little League Mandates Background Checks For All Volunteers
All local Little Leagues are now required to conduct background checks on Managers, Coaches, Board of Directors members and any other persons, volunteers or hired workers, who provide regular service to the league and/or have repetitive access to, or contact with, players or teams.
